Abstract

In a recent study, Zovirax Cream, a prescription-only topical antiviral agent, was significantly more likely to cause eye-related side effects in women than acyclovir and valacyclovir. The incidence of side effects, although not reported, is likely to be underestimated due to the limited available data and potential confounders. In addition, the incidence of adverse events is also likely to be underestimated, owing to the small number of women studied. In this prospective, randomized, double-blind, placebo-controlled study, we compared the safety and effectiveness of Zovirax Cream, acyclovir, and valacyclovir with that of Zovirax and a similar combination of acyclovir and valacyclovir for the treatment of recurrent cold sores (CDHS) in preterm neonates, a population of infants who are at high risk for recurrent cold sores. A total of 250 neonates from the preterm neonatal unit of a tertiary care academic center were randomly assigned to either treatment (n = 80) or placebo (n = 80) for 12 weeks, followed by 12 weeks of treatment followed by 10 weeks of maintenance treatment of the same population. Patients in both groups were followed at 2, 12, 24, and 48 weeks post-randomization.

Results

Aciclovir cream, acyclovir, and valacyclovir resulted in significantly more side effects than Zovirax and Zovirax Cream. Aciclovir and valacyclovir were equally as effective in reducing the number of cold sore lesions, as well as improving the appearance of cold sore lesions. Acyclovir was also comparable to valacyclovir, and the most effective treatment was aciclovir cream in decreasing the number of cold sore lesions and improving the appearance of the lesions.

Overall treatment, the incidence of side effects was similar between the aciclovir and valacyclovir groups. Side effects were less severe for aciclovir cream than for Zovirax and Zovirax Cream. The most common adverse events in aciclovir cream and Zovirax Cream were headache, dyspepsia, and abdominal pain. For the combination group, a total of 16 adverse events were reported and more serious in the aciclovir group. Acyclovir was more effective than Zovirax Cream in decreasing the number of cold sore lesions and improving the appearance of the lesions.

Conclusions

Zovirax Cream, a prescription-only topical antiviral agent, is associated with less adverse effects than acyclovir, and the incidence of adverse events is similar in both groups. However, in comparison with acyclovir, the combination of acyclovir and valacyclovir is equally effective in reducing the number of cold sore lesions and improving the appearance of lesions. Acyclovir is currently being studied in preterm neonates, who are at high risk for recurrent cold sores.
